Medicare Levy & Surcharge

The Medicare Levy is a tax paid by Australian residents to cover health care charges.

The levy is payable on taxable income, and is in addition to your income tax. Individuals and families on higher incomes who do not have an appropriate level of private hospital cover may have to pay the Medicare levy surcharge.

Surcharge income threshold

Year Single person
(no dependents)
Family
(up to one dependent)
2010/2011 $77,000 $154,000
2009/2010 $73,000 $146,000
2008/2009 $70,000 $140,000
2007/2008 $50,000 $100,000

The surcharge income threshold for each extra dependent child is $1,500.

The Medicare Levy Surcharge is in addition to the 1.5% Medicare levy. It is calculated at the rate of 1% of:

  • your taxable income
  • your reportable fringe benefits, and
  • any amount on which family trust distribution tax has been paid.

The Medicare levy is normally calculated at 1.5 per cent of your taxable income but this rate can vary depending on the individual's circumstances.

You may be exempt from the levy or it may be reduced if you:

  • Have a low income; or
  • Are a prescribed person.

2 $37,627 $44,267
3 $40,546 $47,701
4 $43,465 $51,135

(1) No Medicare Levy is payable below the Lower Income Limit.

(2) The rate of 10% of the difference between the taxable income and the lower income limit is payable where the taxable income does not exceed the upper income limit.

(3) The full rate of 1.5% levy is applicable above the Upper Income Limit.

If you have more than four dependent children or students, for each additional child or student:

  • the lower income limit increases by $2,919.

  • the upper income limit increases by $3,434.

A Medicare levy calculator is available at the ATO website to help you calculate your financial obligation.
