How does the combination pill effect you? Tylenol on its own has zero effect so what effect does it have in these combination pills of tramadol or oxycodone?

I’m needing to stop taking aspirin prior to surgery, and want to know what other painkillers I can take that won’t cause blood thinning, and therefore increase bruising and bleeding during and after surgery. Is acetaminophin ok? I’ve read that Aleve is also a blood thinner, which is a shame because it works really well and doesn’t keep me up. I can’t take Tramadol because it makes me psychotic. Plus most narcotics like Hydrocodone and Oxycodone leave me wide awake, and pain free, but I’m useless if I don’t sleep, either because of pain, or drugs keeping my awake.
